Sinus Elevation

When an implant is to be placed on the upper jaw, in place of the molar tooth, the maxillary sinus can sit low towards the tooth, making the procedure difficult. There are cases where the height of the bone is too low because the maxillary sinus has descended due to long term neglect of a lost tooth or severe inflammation on the gum and bone. Prior to implant placement, a pre-surgical sinus elevation procedure is required. Sinus elevation is a procedure where the maxillary sinus membrane is delicately lifted to allow bone grafting in its place for the implant. This process can be done simultaneously with implant placement or can be processed 6 to 9 months after sinus elevation Treatment period ranges between 3 to 9 months depending on the height of the existing bone.

Cases that require Sinus Elevation:

Loss of bone may be due to hereditary factors, periodontal disease and neglected tooth decay

  • Those with insufficient height of the bone in the region of the upper molar teeth
  • Those with insufficient density of the bone in the region of the upper molar teeth
  • Patients with poor bone condition

Sinus Elevation

Lateral Approach Technique (Window) – Approaching from the buccal aspect

Expose the buccal bone by incision of the gum

Detach the bone using a bur and elevate the floor of the maxillary sinus

Fill the space with artificial bone and place the detached bone back in place for suturing

Place the implant after the bone has sufficiently formed

Sinus elevation procedure requires a high level of skill and experience.

The maxillary sinus is covered with a thin membrane of 0.5mm in thickness (Schneider membrane), making sinus elevation procedure a difficult process to ensure the membrane is not damaged. As the procedure can be accompanied by pain and edema, a meticulous post-operative care is essential. Patients with infection, cyst, allergic rhinitis and smokers need a more delicate post-operative care.
